Save a copy before you leave

If portal needs are still unclear after the first call, quote a paid technical discovery phase first, then issue the full proposal once the vendor, data flows, login needs, and responsibilities are confirmed.
This questionnaire is for project scoping and pricing. It is not legal or HIPAA compliance advice. For medical privacy, security, advertising, and tracking decisions, the practice should involve its attorney, compliance contact, and technology vendors. Email is not a secure way to send patient information, so please keep patient details out of your answers.
